Core Technical Guarantees

Confidentiality by Design
Strictly enforces RA 10173 (Data Privacy Act of 2012). Clinical notes and Contact Log B (Sensitive/Crisis) records are restricted exclusively to licensed guidance counselors and the primary counselor on record.
Zero-Install Tele-Counseling
Direct peer-to-peer WebRTC encrypted consultations with crystal-clear audio, video, and in-room chat. Operates directly in modern mobile and desktop browsers with no external app downloads.
Verifiable E-Certificates
Official Certificates of Counseling (FM-13) automatically embed the counselor's digital signature and a unique SHA-256 cryptographic verification token with QR code.
ISO 9001:2015 DPM Alignment
Every guidance service is mapped directly to a standardized Documented Procedures Manual (DPM-001 through DPM-007) and supported by 16 official USTP-GCS digital ISO forms.

2. Authentication, Account Verification & Security

Secure role-based access control, OTP recovery, and administrative counselor vetting

Universal Admin Gate

Counselign implements rigorous access control protocols to protect sensitive student psychological records:

1
Student Registration & Onboarding
Students register using their official University Student ID, institutional email, designated College Department (e.g., CITC, CEA, CSM, COT), and password. Accounts are activated immediately so students can begin their intake.
2
Counselor Credential Verification Gate
To prevent unauthorized access to clinical records, newly registered Guidance Counselors cannot sign in immediately. Their accounts enter a pending status. An Institutional Administrator must inspect their PRC license, verify college assignments, and grant approval under Management > View Users.
3
One-Time Password (OTP) Password Recovery
Forgotten password requests dispatch a time-sensitive 6-digit numeric OTP to the registered email address. The code expires in 10 minutes and prevents automated brute-force attempts.
4
Session Security & Role-Based Access Control (RBAC)
Every HTTP request is authenticated via CodeIgniter 4 session filters. Unauthorized attempts to reach /counselor/* or /admin/* routes are instantly terminated with automatic redirects to sign-in.

3. 11-Part Personal Data Sheet (PDS Intake)

Official ISO FM-USTP-GCS-02 digital psychosocial student profiling system

Student Role DPM-001 FM-02

In compliance with DPM-001 (Individual Inventory Service), every university student must complete the digital Personal Data Sheet (PDS) (/student/pds). This provides counselors with the necessary psychosocial context to deliver personalized, ethical care.

Mandatory Intake Requirement: Standard routine appointment scheduling is gated until the student completes all 11 PDS modules (progress reaches 100%). Walk-in crisis interventions can be overridden by a counselor using Emergency Mode.

The 11 PDS Psychosocial Modules

Part # Module Name Key Clinical Data Collected Guidance Importance

4. 4-Step Smart Appointment Booking Engine

Real-time counselor availability sync, automated conflict detection, and consultation slips

Student Role FM-USTP-GCS-01

Students book consultations through the guided 4-step wizard at /student/schedule-appointment:

1
Step 1: Consultation Nature & Counselor Selection
Choose the primary nature of concern (Academic Difficulties, Career Direction, Personal / Emotional Distress, Family Conflict, Mental Health Support, or Referral Follow-Up). Select your preferred counselor or choose your college's assigned guidance specialist.
2
Step 2: Consultation Mode & Triage Level
Select your consultation format:
  • In-Person (Guidance Center Office): Meet physically in a private counseling booth.
  • Encrypted Tele-Counseling (WebRTC Online): Connect via browser video/audio from a private location.
Toggle the Emergency / Acute Distress Flag if you are experiencing severe anxiety or distress requiring immediate triage.
3
Step 3: Interactive Date & Time Slot Matrix
Select an available date on the calendar. The system fetches the counselor's live 30-minute operational matrix in real time, automatically hiding past times, existing bookings, and departmental blackout meetings. Enforces an automated 10–15 minute rest buffer between sessions.
4
Step 4: Informed Consent & Confirmation
Review your consultation details, read and acknowledge FM-USTP-GCS-10 (Informed Consent & Minor Assent), and click "Confirm Appointment". The system generates an official booking record with status pending and unlocks a printable booking confirmation slip.
Appointment Notifications: Whenever a counselor approves, reschedules, or updates your appointment, real-time alerts appear in your student notification bell and email updates are dispatched to your student address.

5. In-Person & WebRTC Tele-Counseling Consultations

Zero-install peer-to-peer encrypted video, audio, and private in-call chat

Student Counselor

Counselign features built-in WebRTC Tele-Counseling to ensure students can access psychological support regardless of physical campus presence:

1-Click Room Entry
When your appointment is in status approved or in_progress, a prominent "Join Video Room" button activates on your appointment card.
Encrypted P2P Media
Media streams flow directly between the student and counselor using DTLS-SRTP encryption. Video and voice data are never recorded or stored on university web servers.
Confidential Text Chat
An integrated in-room text sidebar allows students who may be in shared dorm rooms to type sensitive details silently while listening to their counselor.

Room Etiquette & Privacy Tips

  • Headphones Recommended: Wear headphones to ensure audio confidentiality and eliminate microphone echo.
  • Private Setting: Position yourself in a quiet room with minimal foot traffic.
  • Browser Permissions: When prompted, click "Allow" for Camera and Microphone access.

6. Needs Assessment, Exit Interview & My Certificates

Annual guidance surveys, senior exit clearance submissions, and verified certificates

Student Role FM-15 & FM-16

In addition to appointments, students access three vital guidance services in their portal:

1. Student Needs Assessment Survey (FM-USTP-GCS-15)

Administered annually under DPM-002 (Orientation & Information Service), students rate their current challenges across Academic, Career, Personal, Financial, and Health dimensions. Results help the Guidance Office design targeted student wellness programs, mental health caravans, and peer support circles.

2. Exit Interview Form & Clearance (FM-USTP-GCS-16)

Graduating seniors, students transferring to other institutions, and those filing for honorable dismissal access the Exit Interview Portal (/student/exit-interview).

  • Submit reasons for departure (Graduation, Career Placement, Relocation, Academic Transfer).
  • Provide 5-point Likert ratings across university services (Instruction, Guidance Services, Facilities, Career Preparation).
  • Submitting the form forwards your profile to the Counselor Exit Hub for clearance review.

3. My Certificates Hub (`/student/certificates`)

Whenever a counselor completes a formal counseling intervention or approves your exit clearance, the official certificate is issued digitally:

Certificate of Counseling (FM-13)
Proves completion of mandatory or referred counseling sessions. Features counselor signature, issuance date, purpose, and cryptographic SHA-256 token.
Exit Clearance Certificate
Required by the University Registrar for diploma release or honorable dismissal. Instantly downloadable in official landscape format with verifiable QR code.

7. Post-Session CSAT Evaluation & Feedback

Continuous quality enhancement through student satisfaction metrics

Student Role ISO Evaluation

When a counselor marks an appointment as completed, the student receives a prompt to complete the post-session Client Satisfaction Survey (CSAT):

  • Counselor Empathy & Listening: Did you feel heard, understood, and respected?
  • Actionability & Safety: Was an actionable next step or safety coping plan established?
  • Atmosphere & Comfort: Was the physical booth or tele-counseling environment comfortable?
  • Overall Service Rating: 5-point Likert scale (Poor, Fair, Good, Very Good, Excellent).

Responses are aggregated into institutional quality metrics used for accreditation reviews, identifying counselor professional development opportunities, and improving guidance center resources.

8. Counselor Caseload Hub & Appointment Lifecycle

Academic department filtering, bulk approvals, rescheduling, and case notes

Counselor Role DPM-005 Contact Log

Guidance counselors manage daily consultations from the Scheduled Appointments Hub (/counselor/appointments/scheduled):

Multi-Dimensional Caseload Filtering

Counselors can filter students by Academic Course / Department (e.g. BSIT, BSHM, BSA, BSED, Crim, Nursing), Year Level (1st–4th Year), and Status Tabs to manage college-specific caseloads efficiently.

Standard Action Workflows

Single & Bulk Approval
Approve appointments individually or select multiple checkboxes across rows and click "Bulk Approve Selected". All records transition to approved simultaneously.
Rescheduling Modal
If an unavoidable schedule conflict arises, click the Reschedule icon. Select a proposed date and slot, enter an explanatory remark, and submit. Status transitions to rescheduled with an instant student alert.
Clinical Notes Filing
Click "Complete Consultation" to document Chief Complaint, Assessment, Interventions Applied, and Action Plan. Completing the notes locks the consultation into Contact Log A (FM-03).
Missed (No-Show) Tracking
If a student fails to appear within 15 minutes of the scheduled time slot without notice, click "Mark Missed". Missed counters identify disengaged students requiring proactive outreach.

9. DPM-003: Internal Referral Service & Triage Engine

Faculty and staff referral intake, triage assessment, and healthcare escalation

Counselor Role DPM-003 / DPM-004

Under DPM-003 (Referral Service), faculty members, academic advisers, and university personnel can formally endorse students requiring psychological assistance to the Guidance Office (/counselor/referrals):

1
Referral Intake & Note 1 (FM-USTP-GCS-11)
Counselors inspect incoming referral slips detailing the referring faculty, student name, academic course, observed behavioral changes (e.g., sudden drop in grades, chronic absenteeism, withdrawal, crying spells), and initial interventions attempted by the instructor.
2
Triage Assessment & Decision Engine
The counselor evaluates the urgency and clinical scope:
  • Accept into Internal Counseling (DPM-005): The case is scheduled for 1-on-1 counseling, auto-logging into the Appointment Log (FM-01) and Contact Log A (FM-03).
  • Escalate to External Referral (DPM-004): If the student exhibits severe clinical symptoms (e.g., active psychosis, severe clinical depression, substance addiction) exceeding university guidance scope, the counselor initiates FM-USTP-GCS-12 (External Healthcare Referral Form) to coordinate with psychiatric hospitals or medical clinics.
3
Faculty Feedback Loop (FM-USTP-GCS-11 Note 2)
While clinical session details remain confidential under RA 10173, the counselor returns an official acknowledgment to the referring instructor confirming that the student has been contacted and is actively receiving guidance support.

10. DPM-005: Counseling Service & 6-Step Crisis Safety Plans

Active caseload management, Contact Logs A & B, suicide prevention plans, and certificates

Counselor Role DPM-005 FM-03/04/13

The Counseling Service Hub (/counselor/counseling) serves as the primary clinical center for registered guidance counselors. It is organized into three specialized operational segments:

The Three Operational Segments

1. Active Cases
Displays students currently engaged in ongoing counseling. Counselors file FM-03 Contact Log A or FM-04 Contact Log B (restricted access for severe trauma or crisis) and schedule follow-up sessions using FM-14.
2. Crisis Safety Plans
A specialized clinical sub-system for students presenting suicidal ideation, self-harm risks, or severe acute trauma. Allows interactive formulation of the standardized 6-step safety plan.
3. Completed Sessions
Historical archive of concluded consultations with full audit trails, counselor clinical summaries, student CSAT ratings, and issued FM-13 Certificates of Counseling.

The 6-Step Suicide Prevention Safety Plan

Formulated collaboratively between counselor and student during crisis interventions:

  1. Step 1: Warning Signs & Triggers: Specific thoughts, moods, behaviors, or situations signaling a crisis is brewing.
  2. Step 2: Internal Coping Strategies: Grounding exercises, deep breathing, journaling, or activities the student can do alone without contacting others.
  3. Step 3: Social Distractions & Settings: Safe places and friends who provide positive distraction (without necessarily discussing the crisis).
  4. Step 4: Trusted Support Persons: Friends or family members the student can openly ask for direct emotional support.
  5. Step 5: Professionals & Crisis Hotlines: Counselor direct line, University Clinic, and National Center for Mental Health (NCMH Crisis Hotline: 1553).
  6. Step 6: Making the Environment Safe: Collaborative plan to remove or restrict lethal means from the student’s living space.

11. Counselor Digital E-Signature Integration

Interactive HTML5 canvas capture, automated margin trimming, and certificate sync

Counselor Role FM-13 & Clearance

Counselign features an integrated Digital Signature Pad enabling licensed guidance counselors to securely affix their real handwritten signature onto official university documents (/counselor/profile):

1
Affix Signature on Interactive Canvas
Navigate to Profile > Digital Signature Pad. Using a mouse, stylus, or touchscreen device, sign your official signature within the designated capture canvas.
2
Automated Whitespace Trimming (`trimCanvas`)
When you click "Save Signature", Counselign’s client-side bounding box algorithm automatically scans pixel data, crops out all unused blank canvas margins, and normalizes the stroke. This guarantees crisp, perfectly centered signature placement without unsightly whitespace borders.
3
Automated Certificate Synchronization
Your trimmed digital signature is stored in the database (counselors.signature_data) and automatically overlays onto:
  • Certificate of Counseling (FM-USTP-GCS-13)
  • Psychometric Test Reports (FM-USTP-GCS-09)
  • Contact Logs & External Referral Letters (FM-USTP-GCS-12)
Any certificate generated with your signature embeds a cryptographic SHA-256 verification token and QR code for registrar verification.
Retroactive Certificate Sync: If you update or re-sign your signature in your profile, the system automatically refreshes your stored signature token across all your verified certificates.

12. DPM-006: Testing Service, DTS Tracking & Batch Scoring

Batch scheduling, live examinee check-in, RPm scoring, and psychological profiles

Psychometrician DPM-006 FM-08/09

In accordance with DPM-006 (Testing Service), psychometric assessments are managed end-to-end via the Testing Service Hub (/counselor/testing):

The Testing Batch Lifecycle

DTS Tracking Code
Every scheduled testing batch generates a unique Document Tracking System code (e.g. DTS-20261023-A4F9) to monitor paper test booklets, answer sheets, and psychometric logs.
Live Hall Check-in
On the examination day, proctors verify student identities and toggle live attendance check-ins inside the testing hall to record who was present or absent.
Psychometric Scoring
Registered Psychometricians input raw scores (e.g. Raven's Standard Progressive Matrices - RPm, 16PF, CFIT). Counselign calculates percentiles, stanines, and descriptive interpretations automatically.
ISO Forms Release
Releasing the batch updates FM-07 (Testing Evaluation), FM-08 (Psychological Test Profile), and FM-09 (Psychometric Report) directly into the student's inventory.

13. DPM-007: Exit Interview & Clearance Management

Senior exit interview review, institutional feedback analysis, and confidential transition guidance

Counselor Role DPM-007 FM-16

Under DPM-007 (Exit Interview & Clearance Service), counselors process graduation and transfer clearances through the Exit Interview Hub (/counselor/exit-interview):

1
Review Student Submission (FM-USTP-GCS-16)
Counselors open the student's submitted exit form. Inspect reason for leaving, future career placement or transfer plans, and university service evaluations.
2
Analyze 5-Category Institutional Feedback
Review student ratings across 5 key dimensions:
  • Quality of Academic Instruction
  • Guidance & Student Well-Being Services
  • Physical Facilities & Laboratory Equipment
  • Career Guidance & Placement Preparedness
  • Overall Campus Experience & Student Climate
3
Encode Counselor Transition Remarks & Approve
Enter concluding counselor transition notes (automatically mirrored to FM-03 Contact Log A) and click "Approve & Sign Clearance".
4
Conclude Exit Interview Session (Step 4)
The system stamps the clearance record with the counselor's trimmed digital signature, generates an official SHA-256 verification hash, and enables instant PDF printing for the University Registrar.

14. Availability Matrix, Blackout Blocks & Crisis Override

Weekly 30-minute interval matrix, meeting blackouts, and emergency bypass

Counselor Role Emergency Override

1. Weekly Recurring Slot Matrix

Counselors configure their standard duty hours via Profile > Availability (/counselor/profile):

  • Toggle Intervals: Enable or disable specific 30-minute blocks (e.g. 8:00 AM-11:30 AM, lunch off, 1:00 PM-4:30 PM).
  • Daily Appointment Cap: Enforce a ceiling (e.g. maximum 6 consultations per day) to maintain quality of clinical care and prevent counselor burnout.
  • Automated 10–15 Minute Buffer: An automated rest interval is placed between consecutive bookings, allowing you to conclude case notes before the next client arrives.

2. Calendar Meeting & Blackout Overrides

When attending official university functions, faculty meetings, academic conferences, or emergency leaves:

  1. Open the Availability Calendar View and select the target date.
  2. Click "Add Meeting / Schedule Blackout".
  3. Select category (Faculty Meeting, Academic Seminar, Campus Assembly, Official Leave) and time range.
  4. Click "Save Blackout": Affected 30-minute slots are instantly disabled on the student booking calendar.
  5. Conflict Detection Engine: If students have already reserved slots within that blackout period, the system flags the collision and activates the 1-Click Reschedule Assistant to reassign affected students.

3. Emergency Booking Override (Crisis Bypass)

In acute psychological crises (severe panic attack, suicidal ideation, traumatic event), walk-in students cannot wait for regular slot openings, PDS completion, or multi-step booking wizards:

How to Execute Emergency Override: Click the red Emergency Button (shield icon) in the counselor navigation bar. Select or search the student, record mandatory clinical remarks (e.g., "Walk-in acute distress at guidance center"), and click "Execute Emergency Booking". The appointment is approved instantly, bypassing all pending limits and PDS gates with an immutable audit record (action = 'emergency_booking_override').

15. Problem-Based Group Counseling Circles

Smart candidate matching, counselor room locks, and dual-layer clinical notes

Counselor Role DPM-005 Group

Group counseling brings students experiencing shared developmental or life challenges together into counselor-facilitated circles (/counselor/group-counseling):

Problem Themes
Select core concerns: Academic Stress, Family Expectations, Career Confusion, Peer Relationships, Grief/Loss, or Working Student Hardship.
Smart Candidate Pool
The system automatically compiles students who indicated this specific concern on their PDS intake or booking notes. Invite 5 to 10 participants with 1 click.
Counselor Start Override
Counselors have complete authority to start the session at any time, even if only 2 or 3 students are present. Click "Lock Room" to prevent late disruptions.

Dual-Layer Documentation

At the end of a group session, the counselor completes two distinct documentation layers:

  • Group Session Summary: General circle dynamics, communication patterns, and group cohesion.
  • Private Individual Notes: Confidential 1-on-1 observations saved exclusively to each student’s permanent record.
  • 1-Click Follow-Up Flag: If any student in the circle displays signs of acute distress, click "Flag for 1-on-1 Follow-Up" to immediately schedule an individual consultation.

16. Institutional Administrator & Accreditation Analytics

Real-time campus dashboards, CHED / ALCUCOA reports, and CSAT survey builders

Admin Role CHED / ALCUCOA

Institutional Administrators oversee campus-wide guidance performance, credential compliance, and quality governance (/admin/dashboard):

Accreditation Analytics & Report Generation

View real-time campus metrics across All Time, Monthly, Weekly, and Daily intervals:

20. Troubleshooting, Deployment & Frequently Asked Questions

Hostinger production setup, signature visibility, browser permissions, and technical FAQs

All Roles Deployment

Hostinger / Production Cloud Deployment Guide

When hosting Counselign on cloud environments like Hostinger cPanel where web assets are split into public_html and backend code is kept in a separate root directory (e.g. /home/u141620581/domains/ustpcounselign.com/Counselign/):

Hostinger SSH Terminal (Port 65002)
$ cd ~/domains/ustpcounselign.com/Counselign
$ composer install --no-dev --optimize-autoloader
$ chmod -R 775 writable/
SSH Terminal Password Tip: On Linux/Unix SSH terminals (including Hostinger's web console), passwords are typed silently. No asterisks, dots, or characters appear on the screen as you type. Type your password completely and press Enter.

The vendor/ directory is ignored by Git by default (via .gitignore). On your server, open SSH or Hostinger Terminal, navigate into the Counselign/ project folder, and run:

composer install --no-dev --optimize-autoloader

Also verify that public_html/index.php correctly points to ../Counselign/app/Config/Paths.php.

Ensure you have recorded and saved your signature under Profile > Digital Signature Pad (/counselor/profile). Counselign uses an automated trimming algorithm (trimCanvas) that crops empty border pixels before saving. Once saved, any new certificate will automatically display your signature. If your browser cached an older certificate view, perform a hard refresh (Ctrl + F5 or Cmd + Shift + R).

Under DPM-001, university policy requires an initial psychosocial intake before booking standard routine consultations. Navigate to Profile > PDS (/student/pds), fill in all 11 required modules, verify completion status shows 100%, and return to the booking engine.

A time slot is disabled if another student has already reserved it, if the counselor has placed a blackout block for an official meeting or seminar, or if it falls outside counselor duty hours. Select any alternate slot highlighted in blue.

When joining a call, your browser displays a permission prompt in the address bar. Click "Allow". If accidentally blocked, click the padlock/tune icon on the left of your browser URL bar, change Camera and Microphone to "Allow", and reload the page.

Click the red Emergency Override Button (shield icon) in the counselor navigation bar. Search or enter the student's name, input the mandatory crisis reason, and submit. The appointment is approved instantly, bypassing all pending limits and PDS gates with an audit log record.

To protect sensitive student psychological records, counselor accounts require administrative verification. An Institutional Administrator must navigate to Management > View Users (/admin/counselors), verify the counselor's credentials, and click "Approve".

CodeIgniter 4 routes dynamic requests through clean controller endpoints. In Counselign, routes are configured in app/Config/Routes.php:

$routes->get('/guide', 'Guide::index');

$routes->get('/guide.html', 'Guide::index');

Both /guide and /guide.html route seamlessly to the user guide controller view.
